Ordeal registration form
Candidates for membership must register 30 days in advance for an Ordeal.

The Ordeal registration form is not available on this web site.  It was mailed after April 1 to elected Scouts and nominated adults who have been approved by the selection committee.  If you do not receive your letter by May 1 or if you have misplaced the form sent to you, contact the Operations Adviser.

2011 Fall Service Weekend
THREE forms are REQUIRED to register for the Fall Service Weekend and/or Brotherhood Quest.  These three forms and your payment MUST BE POSTMARKED NO LATER than SEPTEMBER 16, 2011.

  • Part B of BSA's "Annual Health and Medical Record"  SIGNATURE IS REQUIRED on Part B form.

  The BSA revised the medical form in 2011.  You may use either the 2009 or 2011 medical form to register for the Spring Service Weekend.

  • If you use the 2011 medical form, fill out Part A and Part B and sign Part B.  SIGNATURE IS REQUIRED.
