2013 Spring Service Weekend
THREE forms are REQUIRED to register for the Spring Service Weekend and/or Brotherhood Quest.

Part A of BSA's "Annual Health and Medical Record"
- Part B of BSA's "Annual Health and Medical Record" SIGNATURE IS REQUIRED on Part B.
These forms may be filled out on your computer using the fillable pdf
format by clicking the link above and typing the information onto the text fields. If you use your computer to fill
out these forms, it is helpful to click

easier to see the required information. If you are unable to fill in the medical forms, try updating to the

If you do not want to complete the FILLABLE pdf on your computer, one

Do NOT use this Spring Service Weekend registration form to register for the Ordeal. The Ordeal registration form was mailed after April 1 to
elected Scouts and nominated adults who have been approved by the
selection committee. If you do not receive your letter by May 1 or if you have misplaced the form sent to you,
contact the Operations Adviser.
Those who paid for the 2013 Golden Beaver Lodge Membership MUST submit a registration form and medical form so there will be enough food, but no payment is needed.
The Spring Service Weekend registration form is also in the April - June lodge newsletter.
2013 Section NE-2B Conclave
Register online on the Section NE-2B web site at http://ne2b.org/.
The registration, training, and medical forms that may be printed, filled out, and mailed to register for the Section NE-2B Conclave will be on the Section NE-2B web site when they become available.
2013 Ktemaque Lodge membership
Choose either the 2013 Golden Beaver Lodge Membership form (FILLABLE pdf) or the 2013 Lodge Dues Payment Form (FILLABLE pdf). Payment is DUE by January 1, 2013. The 2013 Golden Beaver Lodge Membership form is also in the lodge newsletter October - December 2012 issue.
If you plan to attend the 2013 Lodge Banquet and be a 2013 Golden Beaver Lodge Member, please mail the 2013
Golden Beaver Lodge Membership form and payment by December 15,
2012 with the 2013 Lodge Banquet Registration form. The fee for
the 2013 Lodge Banquet is included in the fee for the 2013 Golden
Beaver Lodge Membership, so please send in the Lodge Banquet
Registration form and mark on the banquet form that you have paid for
the 2013 Golden Beaver Lodge membership.

Candidates for membership must register 30 days in advance for an Ordeal.
The Ordeal registration form is not available on this web site. It was mailed after April 1 to

If you are a 2013 Golden Beaver Lodge Member and plan to attend the Lodge Fellowship Weekend, you MUST send
in the Fellowship Weekend registration form. Do not send
payment with the Fellowship Weekend registration form since the fee to attend the
Fellowship Weekend was included in the Golden Beaver Lodge Membership
fee. Please contact Mr. Bob F., Membership Adviser, if you have questions.
