2010 Fall Service Weekend
THREE forms are REQUIRED to register for the Fall Service Weekend and/or Brotherhood Quest.  These three forms and your payment MUST BE POSTMARKED NO LATER than SEPTEMBER 17, 2010.
These forms may be filled out on your computer using the fillable pdf by clicking the links above and typing the information onto the text fields and clicking on the buttons and boxes.  If you use your computer to fill out these forms, it is helpful to click "Highlight Fields" on the right side of the purple bar in the upper right corner of the form to make it easier to see the required information.  If you are unable to fill in the medical forms, try updating to the latest version of Adobe Reader at http://get.adobe.com/reader/.

2010 Dues Payment Form  (.pdf) 
The DEADLINE is December 31, 2009 to send your dues form and payment.   This form is also in the Beaver Tracks October - December 2009 issue, which was mailed to all dues-paid members.

Brotherhood registration form
Use the registration form for the Fellowship Weekend, Spring Service Weekend, or the Fall Service Weekend.


Ordeal registration form
The Ordeal registration form is not available on this web site.  It was mailed after April 1 to elected Scouts and nominated adults who have been approved by the selection committee.  If you do not receive your letter by May 1 or if you have misplaced the form sent to you, contact the Operations Adviser.


BSA Annual Health and Medical Record form   This form is in FILLABLE pdf format.  It is helpful to click "Highlight Fields" on the right side of the purple bar in the upper right corner to make it easier to see the required information.  If you are unable to fill in the form, try updating to the latest version of Adobe Reader at http://get.adobe.com/reader/.
  • OA weekend events require completion of Part A on page 2 and Part C on page 4.  Part B on page 3 does not need to be completed for OA weekend events.
  • Signature is REQUIRED on page 4
  • Remember to attach a copy of your medical insurance card (front and back) to Part C on page 4.

2009 Lodge Leadership Development Conference

THREE forms are required to register for the Section Conclave.
